Do your travel plans include you traveling with your pets? Or are your pets staying at a boarding facility, or with a pet sitter, friend or neighbor? Now is a good idea to notify us so we have pertinent information on file in the event your pets get lost or found.
Please fill out the following form. It is important that you provide us with as much information as possible. By doing so, you make it easier for us to locate you should your pet be found.
This information will be discarded 24 hours after the designated end of your vacation. Information supplied to CCTR will NOT be used in any manner other than as it pertains to the CCTR Tag Program. Your information will only be given out on a “need to know” basis, in the event your dog has been lost or found.
