Please provide information about any other medications that the adoptive dog is currently taking, including brand/generic name, dosage, and instructions for administering it (e.g,, time of day, whether to give with food, etc.). Indicate if this medication is for a chronic condition (i.e., dog will need to take it indefinitely, under the supervision of the dog's vet). Also indicate how many doses will be sent with the dog (e.g., 10-day supply).
If the dog is being flown by commercial airline OR being shipped via commercial overland pet transport, what is the ENTIRE cost of the required Health Certificate? Please include the cost of the office exam AND the Health Certificate (ask for quote from the foster dog's vet to determine costs of exam and certificate).
