CCTR: Foster Home Contract Information

Please briefly indicate any particular concerns that need to be included in the contract (examples might include: medication needed to control chronic condition, dog is a digger, dog needs to be fed separately from other dogs, needs obedience training, crawls under fences, doesn't like riding in cars, not completely housebroken, frightened of thunderstorms, etc.).

Please provide information about what heartworm preventative has been used for the dog: brand, dosage (e.g., up to 25 lbs.), when last administered, and number of doses to be sent with dog (if any).

Please provide information about what flea/tick preventative has been used for the dog: brand, dosage (e.g., up to 25 lbs.), when last administered, and number of doses to be sent with dog (if any).

Please provide information about any other medications that the adoptive dog is currently taking, including brand/generic name, dosage, and instructions for administering it (e.g,, time of day, whether to give with food, etc.), Indicate if this medication is for a chronic condition (i.e., dog will need to take it indefinitely, under the supervision of the dog's vet). Also indicate how many doses will be sent with the dog (e.g., 10-day supply).
